Your withdrawal request must be submitted through a form at this time. Please download the appropriate form for the type of withdrawal you wish to make:

• Download Hardship Withdrawal Form 6663
• Download Money Purchase Pension or FLAP Form 7487
• Download Withdrawal and Rollover Requests Form 7126

Once you complete and sign the appropriate form, please mail the form and any additional documentation to the following address:

Alternatively, you can email the completed form and any additional documentation as a PDF file, from the email address we have on file for you, to WPC@mutualofamerica.com. We will only be able to process emailed forms that are in PDF format. Additionally, if you email the form to us using an email address other than the email address we have on file for you, we will unfortunately have to reject your request. We do this to safeguard your account and to prevent fraudulent withdrawals from your account.

If you send your form and any additional documentation to us in an email that is not encrypted, your information is not secure during transmission. This means that if an individual who intends to steal your personal information intercepts the email, they could read or copy the information you provided and use your information for an illegal purpose.
